NCAA Bylaw 11 places limitations on the number and duties of coaches/staff in all sport programs. Each July, the Compliance Office will distribute the Coaching Staff Declaration Form, upon which each head coach will designate his/her staff members for the academic year. Should changes to your staff occur, an updated Staff Declaration reflecting these changes must be submitted to the Compliance Office.

Student-athletes are allowed to participate in legitimate charitable or educational fund-raising and promotional activities. Any participation by student-athletes in such activities must adhere to NCAA regulations. The Promotional Activities Form must be submitted to the Compliance Office for approval before the promotional activity takes place.

+ Educational Video Waiver
Educational Video Waiver

If a student-athlete wants to participate in an educational video related to sport skill instruction, or if a coach wishes to use one of his/her current student-athletes in such a video, an Educational Video Waiver and information concerning the video must be submitted to the Compliance Office.


Bylaw 13: Recruiting

Recruiting Records:
+ Telephone Logs

All Head and Assistant Coaches must maintain an accurate record of all telephone calls and text messages (if applicable) with prospects using one of the proper Compliance Office Telephone Logs. Sports using online recruiting services may submit telephone logs using a print out from the service.

All Telephone Logs must be submitted to the Compliance Office on the first of each month regardless of whether any telephone calls or text messages occurred during that particular month.

Sports that have unlimited phone calls or text messages need to record only the first contact with a prospect.


+ Contact/Evaluation Logs

All Head and Assistant Coaches must maintain an accurate record of all contacts with and evaluations of prospects using the applicable Compliance Office Contact/Evaluation Log. Sports using online recruiting services may submit contact/evaluation logs using a print out from the service.

All Contact/Evaluation Logs must be submitted to the Compliance Office upon return to campus from a recruiting trip. If a reimbursement is requested, Contact/Evaluation Logs must be attached to the Travel Claim submitted to the Business Office. Reimbursements for recruiting trips will not be processed without appropriate recruiting records attached.


+ Record of Recruiting Person Days (M/W Basketball)

In men's and women's basketball, a recruiting-person day is defined as one coach engaged in an off-campus recruiting activity of a prospect on one day; two coaches engaged in recruiting activities on the same day shall use two recruiting-person days. Men's basketball staff members shall not exceed 130 recruiting-person days. Women's basketball staff members shall not exceed 100 recruiting-person days.

The Record of Recruiting Person Days must be maintained in the sports of men's and women's basketball throughout the academic year and submitted on the first of each month to the Compliance Office.


+ Record of Evaluation Days (Football, Softball, Volleyball)
An evaluation day is defined as one coach engaged in the evaluation of any prospect on one day; two coaches making evaluations on the same day shall use two evaluation days. The combined total of such days for all staff members shall not exceed 42 in the fall football evaluation period and 168 in the spring football evaluation period, 50 in softball during the academic year and 80 in women's volleyball during the academic year.

Football ' The Record of Fall Football Evaluation Days must be submitted on the first of the months of October, November and December. The Record of Spring Football Evaluation Days must be submitted on the first of the months of May and June.


+ Unofficial Visit Log

Unofficial visits may be made at any time except during a Dead Period (with some exceptions), including the time period before the prospect's senior year in high school. A record of the unofficial visits that take place in each sport must be maintained on the Unofficial Visit Log and submitted to the Compliance Office at the beginning of each month.


Official Visits:
+ Official Visit Policy

The San Jose State University (SJSU) Athletic Department will conduct all official paid visits in accordance with the guidelines set forth within this Policy to ensure compliance with NCAA Bylaws 13.5 & 13.6. All NCAA, SJSU, and affiliated conferences rules and regulations apply to each official visit and shall be adhered to at all times. The Official Visit Policy is meant to serve as a guideline for coaches, student-athletes, prospects, staff and all others involved in official visits.


+ Official Visit Approval

At least 48 hours prior to the start of the visit, an Official Visit Approval Form, along with a copy of the prospect's transcript, test scores and official visit itinerary must be submitted to the Compliance Office. Approval of the official visit is communicated via email to the sport coaching staff and Business Office. The Compliance Office approval email must be received prior to arranging transportation or lodging for the visit.


+ Student-Athlete Host

The Student-Athlete Host Form must be reviewed with the designated host by a member of the coaching staff prior to the start of the official visit. The Student-Athlete Host Form must be signed by a coach and the host and submitted to the Business Office, along with a Travel Claim at the conclusion of the visit for reimbursement of the host money.


+ Prospect Declaration

Once the prospect arrives on-campus for an official visit, a member of the coaching staff must review with the prospect the Prospect Declaration Form, which outlines the behavioral expectations of the prospect during the visit and NCAA rules governing official visits. The prospect must verify that all NCAA rules were observed during his/her visit by signing the Prospect Declaration Form at the conclusion of the visit. The Prospect Declaration Form must also be signed by a member of the coaching staff and submitted to the Compliance Office.

Meals and lodging may be provided while the prospect is in transit to and from the official visit. The prospect and SJSU coaching staff must complete the Official Visit Travel Voucher and submit it, along with a Travel Claim to the Business Office for reimbursement of such meals and/or lodging.

High school/two-year college coaches are permitted to receive two complimentary tickets, and prospects on official or unofficial visits are permitted to receive three complimentary tickets provided that they are placed on the Complimentary Admissions Request Form. This form must be submitted to the Compliance Office no later than 48 hours prior to the start of the contest for which complimentary admission is requested.

The Camp/Clinic Director must complete the Camp/Clinic Request Form for ALL camps/clinics to be conducted in their sport, including those conducted for individuals who are not of prospect age and those camps/clinics held off-campus.

Upon completion, submit the Camp/Clinic Request Form(s), along with all brochures and/or other methods of advertisement for the camp/clinic, to the Compliance Office for approval at least 4 weeks prior to camp.

The Camp/Clinic Director must complete the Camp/Clinic Discount Form and submit it with the Camp/Clinic Request Form only if there will be any individual or group discounts offered at an SJSU Camp/Clinic.

Free or Reduced Admissions & Refund Roster

A separate Free or Reduced Admissions & Refund Roster must be completed for each camp/clinic in which free or reduced admission is provided. Every participant to be admitted into the camp/clinic free or at a reduced price must be listed.

The Free or Reduced Admissions & Refund Roster must be signed by the Camp/Clinic Director and submitted to the Compliance Office at least one week prior to the start of the camp. If refunds occur after the conclusion of the camp, a separate Roster may be submitted after the camp indicating refunds provided.

The name(s) of each employee who will be employed at the camp/clinic must be listed on the Employment List, their current employment status (e.g., high school/college coach, student-athlete, friend, volunteer), the pay scale the individual will receive on an hourly or weekly basis, and the duties the individual will perform at the camp (e.g., counselor, concessions, officiating, coaching).

The Non-Institutional/Privately Owned Camp/Clinic Employment Form must be submitted to the Compliance Office for approval at least two weeks prior to the start of the camp/clinic.

A Student-Athlete Athletics Employment Form must be completed by all student-athletes employed at a camp/clinic. The Student-Athlete Athletics Employment Form must be submitted to the Compliance Office prior to the start of the camp/clinic. Only one Student-Athlete Athletics Employment Form is required annually for each student-athlete working camps/clinics owned or operated by SJSU or an SJSU staff member.

The Intent to Enroll Form must be completed and submitted to the Compliance Office to lift recruiting and publicity restrictions for a prospect who has not signed an NLI or GIA.


+ Pre-Enrollment Verification

All prospects who intend to use Institutional facilities prior to initial full-time enrollment (incoming freshman and transfers), regardless of summer session enrollment, must complete and submit the Pre-Enrollment Verification Form to the Compliance Office. After medical clearance, the Compliance Office will verify that prospects and student-athletes meet all applicable eligibility requirements for summer athletic activities participation. Prospects and student-athletes are eligible for summer athletic activities only upon receipt of approval via email by the Compliance Office.

+ Local Sports Club

SJSU coaches must report all affiliations with local sports clubs on the Local Sports Club Form, which must be submitted to the Compliance Office prior to participation (includes coaching, administrative duties, tryouts) with such clubs. This form is to be completed once the coach begins duties with thei specific club team and submitted to the Assistant Director of Compliance.


+ On-Campus Evaluation (OCE)

For basketball ONLY, prior to holding an On-Campus Evaluation (OCE) the coaching staff must contact the Compliance Office before the OCE takes place. The OCE Approval Form must be completed by all prospects participating, signed by the sport head coach and submitted to the Compliance Office. The OCE Approval Packet and medical clearance from Athletic Training must also be completed by the prospect and submitted to Athletic Training prior to the prospect's participation in any OCE activities.

A Change of Status Form MUST be submitted to the Compliance Office by a member of the coaching staff for any student-athlete to be added or deleted from a sport Roster. Do NOT provide this Form to a student-athlete to deliver.

+ Travel Roster

At least 24 hours prior to a team's departure for a road trip, the coaching staff must submit a completed Travel Roster to the Compliance Office. The Compliance Office will ensure that all student-athletes traveling are eligible to compete, that departure and return times adhere to NCAA rules, and that travel and squad size limitations satisfy conference regulations.

A completed Transcript Evaluation Request Form must be submitted with ALL transcripts submitted for evaluation. Transcripts submitted without this form attached will be returned. The Transcript Evaluation process begins with the Director of Compliance.

Bylaw 15: Financial Aid

The GIA Renewal/Nonrenewal Policy must be followed to renew or not renew the athletics aid awards of all student-athletes who have eligibility remaining in accordance with Bylaw 15.3.5.1.

+ Athletic Aid Appeals

In accordance with NCAA Bylaws 15.3.2.4, 15.3.4 and 15.3.5, when a student-athlete's athletics aid is reduced or cancelled during the period of the award, or is reduced or not renewed for the following academic year, San Jose State University (SJSU) must provide the student-athlete with an opportunity for a hearing upon the student-athlete's request. The appeal hearing will be conducted by a Subcommittee consisting of the Chair, the Director of Financial Aid, and two additional SJSU campus staff members.

+ Scholarship Textbooks

The Scholarship Textbook Policy is the process used for ordering and obtaining required course-related textbooks/materials/supplies for student-athletes receiving an athletics aid award that includes these items.

+ Student-Athlete Employment

The Student-Athlete Employment Form must be completed by all student-athletes who will be employed while an active member of an NCAA-sponsored sport at SJSU. The Form must be signed by the employer and head coach and submitted to the Compliance Office.

+ Medical Non Counter

If an incapacitating injury or illness results in a scholarship student-athlete's inability to compete ever again, the student-athlete shall not be counted within the institution's maximum financial aid award limitations provided the Medical Non-Counter Policy is followed.

+ Outside Aid Form

Outside Aid Form

NCAA rules require the University to collect information about the outside awards given to student-athletes. This form collects the needed information. Within 10 days of receipt, you and other designated individuals, as indicated, must complete each section of this form. Please return the completed form by fax or email to the Director of Compliance.

In accordance with Bylaw 15.2.6.3, A student-athlete may receive financial aid through an established and continuing program to aid students, provided:
(a) The recipient's choice of institutions is not restricted by the donor of the aid;
(b) There is no direct connection between the donor and the student-athlete's institution; and
(c) The financial aid is not provided by an outside sports team or organization that conducts a competitive sports program to an individual who is or has been a member of that team or organization.

+ Complimentary Admissions
Complimentary Admissions Policy

In all sports in which admission is charged, participating student-athletes may designate up to four individuals to receive complimentary admission to the event. Individuals receiving the complimentary admission must present identification at the admission gate in order to receive the complimentary admission. The Complimentary Admissions List must be submitted by the coaching staff to the Compliance Office at least 24 hours prior to the start of the contest. The Compliance Office approves the List and provides it to the Ticket Office


+ Occasional Meals

In accordance with Bylaw 16.11.1.5, student-athletes or an entire team may receive an occasional meal on infrequent and special occasions in one of two ways:

1. An institutional staff member may provide a student-athlete or an entire team an occasional meal under the following conditions:

  • The meal may be provided either on campus, in a restaurant in the locale of the institution or at the staff member's home;
  • Reasonable local transportation may be provided to attend such meals; and
  • An Occasional Meal Request Form must be submitted and approved prior to the meal taking place.

2. A booster may provide a student-athlete or an entire team an occasional meal under the following conditions:

  • The meal may be provided in an individual's home (as opposed to a restaurant), on campus or at a facility that is regularly used for home competition and it may be catered;
  • A booster may provide reasonable local transportation to student-athletes to attend the meal function only if the meal function is at the home of that booster; and
  • An Occasional Meal Request Form must be submitted and approved prior to the meal taking place

At the conclusion of each sport's championship segment, coaches must complete the Annual Participation Awards Form and submit it to the Associate Athletics Director for Compliance for review to ensure compliance with Bylaw 16 awards legislation. If there are any changes, an updated Annual Participation Awards Form must be re-submitted to the Compliance Office.

The Summer Rehab Agreement Form must be completed by any student-athlete receiving actual and necessary expenses related to rehabilitation during the summer in accordance with NCAA Bylaw 16.4.1. Medical documentation to support the necessity of treatment must be submitted with the Form and a treatment log must be maintained throughout the summer.

+ Countable Athletically Related Activities (CARA)

Countable-Athletically Related Activities (CARA) include any required activity with an athletics purpose, involving student-athletes and at the direction of, or supervised by, any member or members of an institution's coaching staff (including strength and conditioning coaches) and must be counted within the weekly and daily limitations of the sport's declared playing season. A CARA Log must be turned in to the Compliance Office each week during the academic year regardless of whether countable hours took place. This Log must be signed by ALL team members.

There are two versions of the CARA Log. Use the .pdf version of the Log if you wish to handwrite and calculate your sport's weekly hours. The excel version of the Log is formatted to calculate your weekly hours for you based on the reported daily hours.

NCAA Bylaw 17.1.3 requires each member institution to determine the playing season for each sponsored sport. In July all head coaches will receive a Declaration of Playing & Practice Season Form. There are three versions of this Form depending on the sport's playing season - 132-Days, 144-Days or Basketball. The sport appropriate form must be completed and submitted to the Compliance Office. Upon approval, the Compliance Office will return the approved Form and playing season calendar to the head coach.

Should changes to the Playing Season occur, an updated Playing Season Declaration reflecting these changes must be submitted to the Compliance Office.

Football Spring Conditioning Declaration

This form must be completed and submitted to the Compliance Office prior to January 1 in order to identify the eight week discretionary period for football between January 1 and the start of the summer conditioning period.

This form must be completed and submitted to the Compliance Office prior to the first scheduled spring practice in order to identify the 34 consecutive day period within which the fifteen spring practice sessions may occur.

This form is to be completed and submitted to the Compliance Office prior to the beginning of the summer conditioning period in order to identify the nine week summer conditioning period and the one week discretionary period, which must occur within the nine week summer conditioning period.

Head Coaches in Football and Basketball must complete and submit the Summer Athletic Activities Designation Form to the Compliance Office prior to summer school to identify the eight week summer activities period outside the academic year.

Pre-Enrollment Verification Form

As set forth in the Summer Activities Clearance Policy below, all prospects who intend to use Institutional facilities prior to initial full-time enrollment (incoming freshman and transfers), regardless of summer session enrollment, must complete and submit the Pre-Enrollment Verification Form to the Compliance Office. After medical clearance, the Compliance Office will verify that prospects and student-athletes meet all applicable eligibility requirements for summer athletic activities participation. Prospects and student-athletes are eligible for summer athletic activities only upon receipt of approval via email by the Compliance Office.

Outside Competition Form

Prior to practicing or competing in an event while not representing SJSU during or between terms of the regular academic year, student-athletes must obtain written approval from the Compliance Office. Failure to complete the Outside Competition Form prior to participating in an outside event is a violation of NCAA rules.

Outside Competition Policy

A student-athlete wishing to participate in outside competition at any time (academic year or summer) must obtain permission from his/her coach and Compliance Office approval via the Outside Competition Form prior to the student-athlete’s participation.

During team meetings held by the Compliance Office in the beginning of the Fall semester and end of the Spring semester, rules and regulations concerning outside competition are reviewed. Student-athletes are informed of the requirement that an Outside Competition Form must be completed prior to participation in any outside competition.

Prior to participation in summer basketball leagues basketball student-athletes must complete and sign the Summer League form. Once the form has been completed, and the signatures of the student-athlete and head coach have been secured, it should be returned to the Compliance Office for approval.

The Football Preseason Replacement Form is utilized when a football student-athlete sustains an injury or illness that prevents further participation in preseason practice activities and may be replaced with another student-athlete without including that individual in the institution's limitation on number of individuals who may participate in preseason practice.
